Insurance
I am now accepting Aetna, Cigna, and BCBS through Wellnite, or Self Pay at the rates listed below.

If you have a different insurance that has out-of-network benefits, and you meet the criteria to receive a mental health diagnosis, I can provide you with a super bill (invoice summary of paid sessions) to submit to your insurance for potential reimbursement. You are not guaranteed any reimbursement, and it depends on your insurance, so you will need to contact them to learn more about your specific plan.

- Income based Sliding Scale Fees are available upon request and approval.
Rates
The rates listed are full rates. You may have qualified for financial assistance, or received a grandfathered rate based on when you began treatment, resulting in a reduced fee. This fee has been discussed and is reflected in your invoices located on the patient portal. All financial assistance will be re-assessed in January of each year and a new Good Faith Estimate will be issued at that time. All rates are subject to periodic increases based on the cost of living increases yearly. If/when rates change, a new GFE will be issued by Clay Counseling Services, PLLC.
There are non-medical charges, as outlined in the Informed Consent for Treatment form, which are not eligible for discounts or Sliding Scale and due to the nature of the unpredictable nature of each client’s needs, will not be included in the GFE.
Payment
I take direct payment from all major credit and debit cards, Health Savings (HSA), and Flexible Spending (FSA) Accounts.
Notes
*Please note CPT codes will only be used for reimbursement claims from your healthcare insurance provider.
*Cancellation within 24 hours of an appointment will be charged the full price of a session.
*Should you fail to schedule an appointment for three consecutive weeks, unless other arrangements have been made in advance, for legal and ethical reasons, I must consider the professional relationship discontinued.
*No Show/No cancellation will result in a charge of the full price of a session.
